Case Planner Supervisor – RTC SS ($62,000 – $65,000)
Are you looking for a leadership role that combines your clinical and staff development skills? Do you have a passion for working with youth, their families, and the systems they may be involved with? Are you willing to do “whatever it takes,” to support staff, residents, and families in overcoming challenges?
The Case Planner Supervisor is critical to the agency, resident youth, and their family’s progress and success by:
- effectively managing crises/conflicts within case management and planning
- work across RTC departments to ensure resident goals are in progress and is committed to success as related to resident goals and outcomes
- assume responsibility for the department and outcomes.
Qualifications:
MSW degree required with a minimum of 2-years of supervisory experience; Prior case management experience; Some child welfare knowledge is a plus

Case Planner – RTC SS ($50,000-$53,000)
Do you have a desire to assist and advocate for some of NYC’s most vulnerable youth? Are you passionate about family preservation? Do you love being a part of a team delivering an evidence-based model?
St. John’s Residence for Boys is an established child welfare agency in Queens, NY, serving young men from age 13-21 years. The Case Planner is a crucial part of the RTC Social Service Department. They provide direct case management oversight while focused on discharge/permanency efforts for youth and families. Field and office work requiring reliable transportation. Occasional evening/weekend hours may be necessary to ensure services and timelines remain actively engaged.
Qualifications:
Minimum BSW or BA degree in a related field with at least one year of child welfare experience; Case management experience preferred.

Integrated Treatment Model (ITM) Manager – RTC ($55,00-$58,500)
Want to make an impact on someone’s life while growing your career? The ITM Manager is responsible for the implementation and success of the Integrated Treatment Model (ITM) as outlined in the program manual.
- Oversee the ITM Liaison, ensuring protocol and integrity to both agency/program policies
- Innovative and intuitive to the needs of the program as related to the youth’s success
- Establish protocol to ensure Liaison/department needs are aligned to the programs as to program development, project management, and training of staff.
The ITM Manager works closely with the ITM Director of Residential Services and RTC staff to assure model fidelity and integration.
Qualifications
Minimum High School Diploma w/2-years of experience in child welfare required, preferably congregate setting – OR – BA degree w/ a minimum of 1-year experience in child welfare; Some supervisory experience with training emphasis is preferred; Knowledge of Cornell TCI and Safe Crisis Management, preferred/physical ability to be trained, Certification in one or both is preferred.

Licensed Practical Nurse – Rockaway ($53,500-$55,000)
If you have a desire to serve and provide care to youth within the medical arena, this is for you. St. John’s has an on-site medical department that provides daily and emergency care to the resident youth who reside at our NSP/RTC programs.
The LPN is a key member of the medical team and helps to ensure compliance with service and treatment within the department. Provides service to resident youth as needed. Serving as the “second in command,” the LPN assumes responsibility for ensuring the distribution of medication and service and maintenance of confidential information is in compliance with HIPPA and agency policy/ procedure.
Qualifications
BA in Nursing required with some medical case management preferred; Experience working with youth/adolescents is a plus.

MST (Multisystemic Therapy) – RTC SS ($62,000 – $65,000)
Do you love working with evidence-based models? If so, St. John’s is seeking an MST Supervisor to join the team with strong leadership and modeling.
The MST supervisor works collaboratively within the social service department providing MST/ITM program with effective implementation of the MST model of treatment. Collaborates with the administrative/case planning/other staff to address support for the program and residents served. The supervisor is responsible for the clinical and administrative oversight of MST staff and cases.
Qualifications:
MSW or MA degree in a related field required, w/a minimum of 1-year of supervisory experience.

MST-FIT Coach – RTC SS ($47,500 – $51,000)
If you are passionate about working with/for vulnerable youth and families, join St. John’s Residence for Boys as an MST-FIT Coach.
The MST-FIT Coach primarily performs case management and administrative duties around the resident youth’s progress toward the transition from the organization, utilizing the MST-FIT model of service.
This role works both within the organization and the community to ensure appropriate resources and services are aligned for the youth’s success. Compliance with ACS/OCFS mandates and oversight as related to the MST-FIT model of service.
Qualifications:
Minimum BSW or BA degree in a relevant field required with two years of professional experience, including some knowledge of child welfare working with at-risk youth; MSW is preferred with a minimum of one year of related experience; Former case management experience is a plus

PYA Specialist – RTC SS ($47,500 – $51,000)
If you have a passion for helping the city’s vulnerable youth prepare for a future where they excel and succeed, you are the PYA Specialist we seek.
The Preparing Youth for Adulthood (PYA) Specialist works closely across the RTC department to include Fair Futures Coach, Recreation Specialist, Case Planners, and specialized RTC staff to coordinate the delivery of services related to independence.
The Specialist ensures youth will be able to achieve established goals of PYA in accordance with NYS OCFS and ACS regulations. Must have effective communication skills to ensure professional and comprehensible information sharing among agency staff and involved members.
Qualifications:
Minimum BSW or BA in a related field plus 2 years of professional experience working with youth, preferably in a congregate setting; Knowledge of community resources a plus.
